Customer Service Representatives
Sea Mar has customer service representatives in each of its medical facilities to help patients find out if they are eligible for certain public benefits programs.
These include Apple Health (formerly Medicaid), Washington Healthplanfinder, General Assistance – Unemployment, Medicare, and Basic Food Program.
If you would like to find out if you are eligible for any of these programs, visit any Sea Mar medical clinic and ask to speak with a customer service representative.

Note: You will need your household' estimated income, Social Security numbers and proof of citizenship (if applicable) for all applying members.
- Medicaid is Apple Health

- In Washington State, Medicaid is called Washington Apple Health.
- Because of the Affordable Care Act (sometimes referred to as "Obamacare"), more people are able to get preventive care, like cancer screenings, treatment for diabetes and high blood pressure, and many other health care services they need to stay healthy.
- When you're ready to apply, you'll need this information:
- • Your household monthly income.
• The Social Security numbers and dates of birth for each member of your household.
• Your immigration information, if that applies to you. Regardless of immigration status pregnant women and children are eligible for Medicaid coverage in Washington State

- What is Medicare?

Medicare is a federal health insurance program that covers elderly and disabled people. For those with low incomes and resources, there are supplemental programs to help pay for Medicare coverage and/or prescription drug costs.
To be eligible for Medicare, you must
- • Be either 65 and older, under age 65 and have one of a specific list of disabilities, or any age and have permanent kidney failure
• Have entered the US lawfully
• Have lived in the US for at least 5 years

- Basic Food Program?

- The US Department of Agriculture (USDA), Supplemental Nutrition Assistance Program (SNAP), called Basic Food in Washington, helps low income people make ends meet by providing monthly benefits to buy food.
- Who is Eligible:
- Washington state residents who meet:
- • Federal program requirements
• Citizenship or alien status requirements- Legal immigrants who are not eligible for federal basic food solely due their immigration status may be eligible for the state Food Assistance Program (FAP).

- Your Basic Food benefits are based on your family size and income.
